Q In February this year I had to go to the doctor, suffering from tingling (pins and needles) in the tips of my fingers and toes and an unsteady walk. My left arm and hand are also weak. Blood tests were done, and a neck X-ray showed cervical spondylosis. I had physiotherapy, which has now finished. The symptoms have improved, but not 100 per cent.

“Wear and tear” of the vertebrae and the discs between them (cervical spondylosis) occurs in everyone eventually. This process narrows the spaces between the sides of the vertebrae through which nerves pass to and from the spinal cord, and once these nerves get pinched you can get tingling in the fingers.

Accompanying symptoms are those common to arthritic joints, such as local pain and stiffness in the neck. The latter might respond to painkillers such as ibuprofen, but there is not much one can do about the tingling except for physiotherapy and the passage of time.

When more severe or more distant symptoms such as arm weakness, tingling in the toes and an unsteady walk are involved, then one has to consider if the spinal canal itself is getting narrow, thus pressing on the main spinal cord. This could still be cervical spondylosis of a greater degree or it could be due to a bulging disc – the same sort of process which in the lower back causes sciatic pain to go down the leg.

The only way to be sure what is going on is to have further tests, such as a CT or MRI scan of the neck, which can display not only the bones but also the softer tissues such as the nerves. Such tests would usually be done by a neurosurgeon.

If all your symptoms are improving, then it is likely no extra treatment would be needed, but you may wish to discuss with your GP if you should be referred.

The cervical spondylosis you speak of is, in the majority of cases, caused by ageing. Many men and women aged over 50 display some signs of this condition. In some cases, it is just the neck that is causing pain and in others, the arms and legs are involved.

In a Dutch study of the condition, those with even the most acute neck pain reported progress after a year, with more than three quarters seeing an improvement (but not a complete lack of pain).

In cases of chronic pain, a referral to a pain-management clinic is probably appropriate, or in cases involving nerve root symptoms, it may be necessary to consider surgery.
